Join our local transformation to a better human service system

 

Curandi believes transformation begins with active care coordination at the point of need, one person or family at a time. But the reality is complex needs for vulnerable people cannot be met with our current fragmented, rigid, and siloed system.

In Marion-Polk counties, Curandi is addressing this challenge by helping grow a robust, local human service network of effective social service agencies. Curandi supports network members with the right tools and technology to foster collaboration, track outcomes,  and make performance-based payment possible.

Members

Members are Marion-Polk human and social service agencies that seek the agility, data, and connections to better address complex and constantly adapting social needs more effectively.

 
 

Catholic Community Services (CCS) is a nonprofit organization in Oregon helping those with intellectual, development disability and children, youth and families facing adversity develop safe, stable nurturing relationships; enjoy good health; gain the knowledge, skills, and virtues they need to succeed; and enjoy financial self-sufficiency.

 
 

Fostering Hope Initiative (FHI) is a neighborhood-based collective impact project in Marion County, Oregon. A collaborative of independent social service and behavioral health providers, FHI gathers the expertise of many agencies together and focuses their efforts on the shared goal of helping families in low-income neighborhoods build skills and connections to combat toxic stress and break the cycle of abuse. This structure has allowed FHI to significantly reduce parental stress levels in its neighborhoods, and is a model example of how collaborative, adaptable networks can improve individuals’ lives, strengthen communities, and reduce the cost of care.

 
 
 

Church @ the Park (C@P) is a nonprofit organization that has been serving our unsheltered neighbors since 2007. C@P seeks the mutual transformation of unsheltered people, our staff, and volunteers through relationships, agency partnerships, and tangible resources. As the Capital of Oregon, Salem is a central hub in the state for those experiencing homelessness, incarceration, and mental illness. C@P seeks to be a peacemaker in our city through the provision of low barrier shelter, person-centered navigation services, and workforce development.

Partners are technical advisors to Curandi, and to funders and other community health advocates invested in transformation to a better human service system.

 
 

Activate Care is a cloud-based platform for precision care coordination that allows care teams to effectively address social determinants of health. Easy to implement and use, Activate Care allows providers, clients, families, and community support to collaborate and drive compliance to shared care plans.

 
 

Pacific Research and Evaluation, LLC helps community providers prove that their programs achieve desired outcomes. Objective and rigorous, their collaborative consultants draw on a variety of backgrounds to research and supply evidence that their clients’ programs are great – and give feedback to make them even better.
